Community Health Service Inc.

Community Health Service Inc. (CHSI) is a network of federally qualified health centers in Minnesota and North Dakota, including clinics in Moorhead, Rochester, Willmar, and Grafton and a victim advocacy services office in Crookston. Community Health Service Inc. provides quality, comprehensive health care at an affordable price — accepting Medicaid, Medicare, private insurance, and sliding fee payments. Our model of integrated services includes medical, dental, and behavioral health care, as well as community referrals, Medicaid enrollment, and advocacy and crisis services for victims of domestic violence and sexual assault.

CHSI strives to empower patients managing chronic conditions and improve health literacy leveraging electronic medical records system, telehealth and cutting-edge technologies. Due to the predominantly rural service areas of North Dakota and Minnesota, CHSI also operates three mobile units that provide health and dental services where patients live and work. CHSI deploys these mobile units to farms, local restaurants, churches, employers or any location where patients do not have transportation or resources to travel.

A Board of Directors, comprised of a 51 percent patient majority, governs CHSI. As a result, patients truly have a voice in guiding and directing their community health.

Lake Superior Community Health Center

Lake Superior Community Health Center (LSCHC) is a 501(c) 3 non-profit organization. Since its inception in 1972, LSCHC has grown and expanded, and today operates as a federally-qualified health center (FQHC), providing comprehensive primary and preventive health care services for the communities in the Twin Ports. We have clinic locations in Duluth, MN and Superior, WI. We treat the whole family in an integrated care model, regardless of insurance or income.

Sawtooth Mountain Clinic

Sawtooth Mountain Clinic, Inc. (SMC) is a Federally Qualified Community Health Center in Grand Marais, MN. Founded by Drs. Roger MacDonald, David Hilfiker, Bill Gallea, and Barbara Bank in 1979, SMC has served the health care needs of Cook County, MN, for 40 years.

Through financial support from the federal government, we are able to offer high-quality primary health care to every patient, regardless of the ability to pay. In addition, a Sliding Fee Scale Program is available to help offset the cost of care at our clinic as well as services offered by the local hospital, dentist office, and a wide range of mental health services.

Scenic Rivers Health Services

Scenic Rivers Health Services is a Community Health Center and a Federally Qualified Health Center. Originally started as Cook Area Health Services in 1979, we’ve been expanding into new clinics and gaining new providers ever since. We currently serve an 8,300 square mile service area with six medical clinics and four dental clinics spread over three counties. Scenic Rivers Health Services is also associated with critical access hospitals in Cook and Bigfork, which allow us to provide patient care 24 hours a day
